- Adenovirus Infection in a Kidney–Pancreatic Transplant Recipient: Case ReportPublication . Most frequently, it occurs in the first year after a kidney transplant, and it is secondary to the reactivation of latent disease. However, primary infection may occur, and disseminated disease is more common when related to primary infection. Kidney involvement may be confirmed by biopsy, although diagnosis may be presumptive. Reduction of immunosuppression and supportive care are important components of therapy. CASE DESCRIPTION: A 41-year-old female renal-pancreatic recipient 12 years before with chronic renal graft dysfunction and a functional pancreatic graft had a history of cytomegalovirus and polyoma virus infection 2 years after transplantation. She was taking tacrolimus, mycophenolate mofetil, and prednisolone. The patient was admitted after persistent uncharacteristic diarrhea 3 weeks before hospitalization without any relevant epidemiologic context. She was dehydrated, and the lab results showed worsened kidney function and leucocytosis. The viral culture revealed adenovirus. Vigorous hydration was implemented, and the mycophenolate mofetil dose was reduced. The patient was discharged, and renal function returned to previous values. DISCUSSION AND CONCLUSION: Adenovirus infection has a wide clinical presentation, and multisystemic involvement may occur in transplant recipients. Supportive care is paramount. The clinical features and viral culture confirm the diagnosis, although tissue samples and quantitative polymerase chain reaction may be required in more severe cases.
- Alergia a Medicamentos Reportada em Crianças que Frequentam InfantáriosPublication . Carreiro-Martins, P; Belo, J; Marques, J; Papoila, AL; Caires, I; Araújo-Martins, J; Pedro, C; Rosado-Pinto, J; Virella, D; Leiria-Pinto, P; Neuparth, NIntrodução: A prevalência de alergia a fármacos na população geral não se encontra devidamente caraterizada, existindo poucos estudos publicados que tenham abordado esta situação em crianças com idades inferior a seis anos de idade. Este estudo tem como objetivo principal estimar a prevalência de alergia a medicamentos reportada pelos pais de crianças de infantários de Lisboa e do Porto. Material e Métodos: No âmbito da Fase II do projeto “ENVIRH – Ambiente e Saúde em Creches e Infantários” foi aplicado um questionário sobre alergia a medicamentos aos pais das crianças, recrutadas por amostragem aleatória estratificada dos infantários. Resultados: Foram analisados 1 169 questionários, 52,5% de rapazes. A idade média foi de 3,5 ± 1,5 anos. A prevalência de alergia a medicamentos reportada foi de 4,1% (IC 95%: 3,0 - 5,2%). Os fármacos mais referidos foram os antibióticos (em 27 reações) e os AINEs (em seis reações). Na análise multivariável, a alergia a medicamentos reportada associou-se diretamente com a idade da criança (OR 1,19; IC 95% 1,01 - 1,41) e com a referência a alergia alimentar (OR 3,19; IC95% 1,41 - 7,19) e inversamente com o nível de escolaridade dos pais (OR 0,25; IC95% 0,10 - 0,59). Discussão: Apesar das limitações do estudo, os resultados encontram-se de acordo com o reportado por outros autores e sugerem que a prevalência reportada de alergia a medicamentos seja elevada no grupo etário estudado. Conclusão: Torna-se necessário que situações de alergia a medicamentos reportadas pelos pais sejam devidamente estudadas, no sentido de evitar evicções desnecessárias que possam condicionar opções terapêuticas em futuras situações de doença.
- Aplicação da Children's Depression Inventory Numa População Clínica de Adolescentes: Estudo ExploratórioPublication . Moreira, AL; Vilariça, P; Marques, J; Pedro, F; Matos, AIntrodução: A Children’s Depression Inventory (CDI; Kovacs, 1992) é usada para avaliar a existência de sintomatologia depressiva na infância e adolescência e tem sido amplamente aplicada em populações não clínicas, em Portugal, desde a sua validação (Marujo, 1994). Porém, os dados referentes a populações clínicas são escassos. Objectivo: Pretende-se estabelecer correlações clínicas numa amostra de adolescentes seguidos em consulta de Pedopsiquiatria fazendo uma contribuição para a validação da CDI em Portugal, com vista a permitir o alargamento da sua aplicação a populações clínicas. Métodos: A CDI -27 itens foi auto-preenchida na 1ª entrevista de atendimento por 35 adolescentes (F=23; M=12) com necessidade de rastreio de depressão, utentes da Consulta de Ambulatório da Clínica da Juventude entre Janeiro/2010 e Julho/2011. Foram excluídos os casos com processo clínico incompleto. Na estatística usou-se o IBM SPSS 19. Resultados: Os dados correspondem a 7,9% da população de origem (N=443). A idade média foi de 14,5 anos (Mín. =13; Máx. =16). A escolaridade média foi de 8,54 anos (Mín. =5; Máx. =11). A estrutura familiar e motivos de pedido de consulta corresponderam aos estudos anteriores efectuados na Clínica. O cutt-off do nível clínico é 15 e 68,5% da amostra estava acima deste valor. O humor negativo (26%) e sentimento de ineficácia (23%) foram as subescalas mais elevadas; 40% dos jovens pontuou para mais do que uma subescala. Os diagnósticos obtidos foram Perturbações do Humor (43%), Perturbações de Adaptação (17%), Perturbações Disruptivas do Comportamento (14%), Perturbações de Ansiedade (9%), Problemas com Grupo de Apoio Primário (3%), Perturbações de Personalidade (3%), e outros (9%). O qui-quadrado não foi significativo para um cut-off de 15 (considerados Perturbações do Humor e Outros diagnósticos). Conclusões: A CDI e os resultados das subescalas são úteis para uma abordagem focalizada e permitem a priorização de casos. Porém, os resultados obtidos comprometem o uso da CDI na detecção de Perturbações do Humor. Os autores sugerem a realização de mais investigações com o objectivo de alargar e melhorar a avaliação do uso clínico da CDI.
- Atypical Phenotype in Two Patients with LAMA2 MutationsPublication . We report two patients with partial laminin-a2 deficiency and atypical phenotypes, one with almost exclusive central nervous system involvement (cognitive impairment and refractory epilepsy) and the second with marked cardiac dysfunction, rigid spine syndrome and limb-girdle weakness. Patients underwent clinical, histopathological, imaging and genetic studies. Both cases have two heterozygous LAMA2 variants sharing a potentially pathogenic missense mutation c.2461A>C (p.Thr821Pro) located in exon 18. Brain MRI was instrumental for the diagnosis, since muscular examination and motor achievements were normal in the first patient and there was a severe cardiac involvement in the second. The clinical phenotype of the patients is markedly different which could in part be explained by the different combination of mutations types (two missense versus a missense and a truncating mutation).
- Cross-Cultural Validation of the Portuguese from Portugal Version of the Test for Respiratory and Asthma Control in Kids (TRACK) QuestionnairePublication . The Test for Respiratory and Asthma Control in Kids (TRACK) is a tool to assess asthma control in preschool children. This study aims to validate the Portuguese from Portugal version of the TRACK questionnaire. Methods. A prospective cohort study was carried out to assess their psychometric characteristics. Caregivers of 141 children under age 5 with asthma symptoms were enrolled. Results. Internal reliability was close to 0.70 (Cronbach's α). The test-retest reliability was 0.87. TRACK scores were different between well, partially, and non-controlled asthma groups (p less than 0.001). Patients rated as having better control showed an increase in TRACK scores. Conclusions. The Portuguese version of the TRACK questionnaire is accurate and reliable for monitoring asthma control. Its use may help to overcome challenges with the management of this age group.
- Doença de Mãos-Pés-Boca no Adulto - Um Caso ClínicoPublication . Amálio, S; La Féria, P; Ávila, P; Matos, R; Faro, T; Mendonça, M; Santos, T; Marques, JA doença de mãos-pés-boca é uma infecção viral, mais frequentemente causada pelos vírus coxsackie A16 e enterovírus 71, que afecta preferencialmente crianças com idade inferior a 10 anos. Geralmente é uma doença benigna, caracterizada por febre e vesículas na boca e extremidades distais, mas também pode associar-se a complicações mais graves com meningoencefalite ou miocardite. Os autores reportam um caso de DMPB num adulto imunocompetente, com 27 anos de idade, que apresentava pápulas das mãos, pés e cavidade oral, algumas com progressão posterior para vesículas. A exuberância do quadro clínico e curso mais prolongado da doença também constituíram características pouco usuais.

Objective: This study aims to explore the association between clinical and functional parameters and the lack of asthma control in preschool children. Methods: Children aged 3-5 years with asthma and healthy controls were recruited. A questionnaire was used to identify potential risk factors for uncontrolled asthma, as defined by the Global INitiative for Asthma criteria. Lung function and bronchial reversibility were evaluated through impulse oscillometry and spirometry. Adjusted odds ratios were estimated based on multivariable generalized additive regression models. The discriminative ability of the models was measured by the area under the receiver operating characteristic curve (AUC). Results: The study population comprised 121 children (107 with asthma and 14 healthy controls). Fifty-three patients (50%) had uncontrolled asthma. After adjustment, the variables associated with an increased risk of lack of control were as follows: “More than 3 flare-ups in the last 12 months”, “Moderate to severe rhinitis”, and “Relative variation in postbronchodilator FVC and FEV1”. The AUC of the final models that included variation in FVC or FEV1 were 0.82 and 0.81, respectively. The R5-20, R5-20%, and AX z-score values of the healthy group were lower than those of children with asthma. Conclusion: In preschool children, clinical and functional parameters are associated with uncontrolled asthma. More studies are needed to confirm the usefulness of impulse oscillometry.
