- Three-Dimensional Contrast-Enhanced Magnetic Resonance Pulmonary Angiography in the Study of Pulmonary Vascular PathologyPublication . Thomas, B; Farto e Abreu, P; Roquette, J; Ferreira, ROBJECTIVES: To assess the feasibility of performing pulmonary angiography using MRI with contrast enhancement in patients with pulmonary vascular disease. METHODS: We present our experience in ten individuals, two controls and eight patients who underwent the exam after injection of a gadolinium-based contrast agent on a 1 Tesla MR scanner using a time-of-flight sequence and breath-holding during injection of contrast. RESULTS: Pathology in the main pulmonary artery and its major branches was detected easily while resolution at the segmental and subsegmental levels was inadequate. CONCLUSION: Contrast-enhanced magnetic resonance pulmonary angiography is feasible on a 1 Tesla MR scanner for the study of pathology of the main pulmonary artery and its major branches, like massive pulmonary embolism. However its ability to detect and define distal vessel pathology as found in chronic thromboembolic pulmonary hypertension and small pulmonary emboli is limited.

Horta, A; Goes, MJ; Oliveira, S; Castro, ASOs A.A apresentam dois casos clínicos de tumores hepáticos associados ao torotraste, dois colangiocarcinomas, observados numa enfermaria de Medicina Interna de um hospital de Lisboa. É feito um comentário sobre o efeito nefasto das radiações ionizantes em geral sobre o organismo humano, particularizando os efeitos do torotraste. Comenta-se, ainda, o prolongado tempo de latência que existiu nestes dois casos, bem como o facto de, provavelmente, estarmos perante os últimos casos de tumores induzidos pelo torotraste.

Coelho, PP; Castelão, N; Roquette, JPurpose: To assess the results obtained in very high-risk patients, which are those patients with an EUROSCORE greater than 13 points. Material and methods: From September 2001 to September 2003, thirty-three very high-risk patients were operated on in our department, which represents 1.6% of all the surgical activity during that period of time, being 17 male and 16 female, with an average of 69 years old (maximum 86 and minimum 32). Diagnosis includes: post infarction CIV 5, coronary insufficiency 11, aortic dissection 3, mitral prosthesis 3, valvular disease 9, aortic prosthesis disfunction 2. Fifteen patients underwent an emergency procedure, 12 were urgent and the remaining 6 were electively operated on. Results: Overall post-operative mortality was 12 patients (36%), being 6 emergent, 5 urgent and 1 elective patient. Patients who survived the operation had longer intensive care and hospital admission periods, which will be analyzed in detail. Conclusion: Surgery can be justified in very high risk patients. Despite the high perioperative mortality and longer periods of hospital stay, they will be otherwise condamned to death, if surgery would not be performed.
- Gravidez Espontânea após Falência Ovárica Prematura - um Caso ClínicoPublication . Grilo, I; Chung, A; Lacerda, A; Serrano, FA falência ovárica prematura (FOP) corresponde à deplecção precoce dos folículos ováricos, antes dos 40 anos, sendo na maior parte dos casos de etiologia desconhecida. Desenvolve-se em 1% das mulheres, sendo responsável por amenorreia, infertilidade, deficiência de hormonas sexuais esteróides e elevação das gonadotrofinas. Metade das mulheres com FOP com cariótipo normal têm folículos ováricos que funcionam intermitentemente e 5 a 10% das mulheres jovens com FOP engravidam espontaneamente. Os autores apresentam um caso clínico de uma doente com 34 anos de idade, que desenvolveu amenorreia aos 31 anos, um ano após o parto. Apresentava persistência de valores elevados para a FSH e LH, e valores no limiar inferior do normal para os estrogénios. Iniciou reposição hormonal com terapêutica estroprogestativa (THS) tendo engravidado espontaneamente durante 6º mês de tratamento. A gravidez e parto descorreram sem intercorrências.

PATIENTS: This phase III open-label study evaluated the virological and biochemical response to lamivudine in 70 Portuguese patients with HBeAg positive chronic hepatitis B. Patients were treated with lamivudine 100mg once daily for 12 months. METHODS: Antiviral activity was assessed by measuring alanine aminotransferase (ALT)/aspartate aminotransferase (AST) levels at all protocol visits, and hepatitis B serology and HBV DNA were performed at baseline and at month 12 visits. Evaluation of safety and tolerance was based on clinical adverse events and laboratory analyses. RESULTS: The primary endpoint was virological response at month 12, defined as loss of detectable HBeAg from serum with a reduction of HBV DNA to undetectable levels, and this was observed in 19/69 (27.5%) of patients. Almost half of the patients were HBV DNA negative by this time. Mean ALT values decreased steadily during treatment and by 12 months 61% of patients had values within the normal range. HBeAg seroconversion (HBeAg negative, HBeAb positive) was achieved in 27.9% of patients by 12 months, although all patients remained HBsAg positive. CONCLUSION: Lamivudine was well tolerated and the incidence of adverse events was similar to those reported in previous studies. Lamivudine treatment resulted in virological and biochemical improvements in HBeAg positive chronic hepatitis B patients, with HBeAg seroconversion in one-third of patients.
- Fexofenadine is Efficacious and Safe in Children (Aged 6-11 Years) with Seasonal Allergic RhinitisPublication . Objective: This randomized, placebo-controlled, parallelgroup,double-blind study was performed to assess the efficacy and safety of fexofenadine in children with seasonal allergic rhinitis. Methods: This study was conducted at 148 centers in 15 countries. Nine hundred thirty-five children (aged 6-11 years) were randomized and treated with either fexofenadine HCl 30 mg (n = 464) or placebo (n = 471) tablets twice a day for 14 days. Individual symptoms (sneezing; rhinorrhea; itchy nose, mouth, throat, and/or ears; itchy, watery, and/or red eyes; and nasal congestion) were assessed at baseline and then daily at 7:00 AM and 7:00 PM (±1 hour) during the double-blind treatment period. Each total symptom score was the sum of all symptoms, excluding nasal congestion. The primary efficacy variable was the change from baseline in the average of the daily 12-hour evening reflective total symptom scores throughout the double-blind treatment. Safety was evaluated from adverse-event reporting, vital signs, physical examinations, and clinical laboratory data at screening and study end point.
- Trombo Livre Intra-Auricular Direito e Embolia PulmonarPublication . Galrinho, A; Cruz, T; Loureiro, JOs autores descrevem um caso de um doente com uma neoplasia da bexiga inoperável, internado por um quadro de síncope e hipotensão arterial. Na avaliação do doente é efectuado um ecocardiograma que mostra a existência de um trombo livre ao nível das cavidades direitas e dilatação das mesmas que levou à hipótese diagnóstica de embolia pulmonar. Perante a existência de uma neoplasia com hemorragia recente, algumas dúvidas terapêuticas surgiram, tendo sido iniciado heparina. Devido a agravamento da situação, com hipertensão pulmonar grave e presença de volumosos trombos ao nível de ambos os ramos da artéria pulmonar visualizados por ecocardiografia transesofágica, o doente acabou por ser submetido a trombólise, embora com algumas alterações ao esquema habitualmente realizado. O doente melhorou, sendo o ecocardiograma final normal.
