- Alcohol Septal Ablation in Obstructive Hypertrophic Cardiomyopathy: Four Years of Experience at a Reference CenterPublication . Fiarresga, A; Cacela, D; Galrinho, A; Ramos, R; Sousa, L; Bernardes, L; Patrício, L; Cruz Ferreira, RINTRODUCTION: We describe our center's initial experience with alcohol septal ablation (ASA) for the treatment of obstructive hypertrophic cardiomyopathy. The procedure, its indications, results and clinical outcomes will be addressed, as will its current position compared to surgical myectomy. OBJECTIVE: To assess the results of ASA in all patients treated in the first four years of activity at our center. METHODS: We retrospectively studied all consecutive and unselected patients treated by ASA between January 2009 and February 2013. RESULTS: In the first four years of experience 40 patients were treated in our center. In three patients (7.5%) the intervention was repeated. Procedural success was 84%. Minor complications occurred in 7.5%. Two patients received a permanent pacemaker for atrioventricular block (6% of those without previous pacemaker). The major complication rate was 5%. There were no in-hospital deaths; during clinical follow-up (22 ± 14 months) cardiovascular mortality was 2.5% and overall mortality was 5%. DISCUSSION AND CONCLUSION: The results presented reflect the initial experience of our center with ASA. The success rate was high and in line with published results, but with room to improve with better patient selection. ASA was shown to be safe, with a low complication rate and no procedure-related mortality. Our experience confirms ASA as a percutaneous alternative to myectomy for the treatment of symptomatic patients with obstructive hypertrophic cardiomyopathy refractory to medical treatment.
- Angioplastia Directa como Terapêutica de Reperfusão no Idoso Acima dos 75 AnosPublication . Feliciano, J; Fiarresga, A; Timóteo, AT; Pelicano, NJ; Cacela, D; Cruz Ferreira, R; Ferreira, ML; Gonçalves, JM; Quininha, JIntrodução: A angioplastia primária é a terapêutica de reperfusão com melhores resultados no enfarte agudo do miocárdio (EAM). Este facto assume particular relevância na população acima dos 75 anos, em que o risco hemorrágico relacionado com a trombólise se associa ao acréscimo de mortalidade condicionado pela idade. Objectivos: Avaliar em doentes com síndrome coronário agudo (SCA) com supradesnivelamento do segmento ST e submetidos a angioplastia directa, a influência da idade nos resultados, incluindo evolução clínica, morbilidade e mortalidade intrahospitalar. População e métodos: Análise retrospectiva de 245 doentes consecutivos admitidos entre Janeiro de 2000 e Dezembro de 2001 por SCA com supradesnivelamento ST, com idades entre os 31 e 90 anos (63+-13), submetidos a angioplastia primária. Destes, 46 doentes (18,8%) com idade superior a 75 anos foram comparados com os restantes. Avaliaram-se os seguintes parâmetros: factores de risco cardiovascular – hipertensão arterial (HTA), diabetes mellitus (DM), tabagismo, dislipidémia, história familiar -, antecedentes de angina ou EAM e angioplastia prévia; extensão de doença coronária, resultados, evolução clínica e mortalidade intrahospitalar. Resultados: Nos doentes com idade superior a 75 anos, o sexo feminino foi predominante (26 doentes, 56,5%), assim como a existência de HTA, angina e DM tipo2, embora sem significado estatístico. O tabagismo foi mais prevalente em idades inferiores (54,3% vs 13%;p<0,001), assim como EAM antigo (p<0,05). A taxa de sucesso primário global foi de 96,7% (93,5% no grupo acima de 75 anos). A doença de 3 vasos foi significativamente mais frequente na população idosa (41,3% vs 26,2%; p<0,05); a utilização de inibidores da glicoproteína IIb/IIIa foi menor (80,4% vs 91%;p<0,05). A evolução em classe clínica Killip (KK) foi mais favorável no grupo com menos de 75 anos (KKI: 88,4% vs 69,8%;p<0,001). Dos eventos estudados, observámos na população idosa maior ocorrência de complicações hemorrágicas minor (3,5% vs 13,0%; p<0,05) e de morte intrahospitalar (4,5% vs 19,6%; p<0,001). Conclusão: A angioplastia primária mantém nesta população uma taxa de sucesso primário semelhante. A maior incidência de complicações hemorrágicas implica um maior cuidado na selecção da terapêutica antitrombótica adjuvante. A mortalidade significativamente mais alta neste grupo etário mantém-se, mesmo quando a angioplastia é o método de reperfusão escolhido.
- Coronary Sinus Reducer Device for the Treatment of Refractory Angina: a Multicenter Initial ExperiencePublication . Ferreira Reis, J; Brízido, C; Madeira, S; Ramos, R; Almeida, M; Cacela, DIntroduction: The coronary sinus Reducer (CSR) device has emerged as a complementary therapy in patients with severe angina refractory to optimal medical therapy and not amenable to revascularization. Our aim was to assess the safety and efficacy of the CSR in a real-world setting. Methods: Twenty-six patients with refractory angina (RA) and evidence of myocardial ischemia attributable to the left coronary artery considered unsuitable for revascularization were treated with the CSR at two centers between May 2017 and July 2019. Safety endpoints were procedural success and complications. Efficacy endpoints, assessed at six-month follow-up, were reduction in CCS class, improvement in quality of life (QoL) assessed using the short version of the Seattle Angina Questionnaire (SAQ-7), and reduction in antianginal therapy. Results: Twenty-three patients had end-stage coronary artery disease without revascularization targets and three had microvascular disease without epicardial stenosis. Procedural success was achieved in 23 patients, with two device/procedure-related complications and one anatomically-related failure to deliver the device. A total of 24 patients had the device implanted and entered the efficacy analysis. Eighteen patients (75.0%) had a reduction of at least one CCS class, 41.7% had a reduction of at least two classes, and 16.7% became asymptomatic, with a mean reduction in CCS class of 1.3±0.2 (p=0.001) at six-month follow-up. All SAQ-7 domains improved, notably physical limitation (p=0.001), angina frequency (p=0.005) and QoL (p=0.006). There was a mean reduction in anti-ischemic drugs from 3.4±1.1 to 2.9±1.2 (p=0.010). Conclusion: In this real-world, multicenter experience, implantation of the CSR was associated with improvement in angina and QoL in patients with RA.
- Existem Diferenças nos Resultados da Angioplastia Directa no Sexo Feminino?Publication . Feliciano, J; Fiarresga, A; Timóteo, AT; Pelicano, NJ; Cacela, D; Cruz Ferreira, R; Ferreira, ML; Gonçalves, JM; Quininha, JIntrodução: Em doentes com enfarte agudo do miocárdio (EAM), a angioplastia percutânea (ACTP) é de forma crescente aceite como a terapêutica de reperfusão mais eficaz. No sexo feminino estão descritos factores com implicação prognóstica na evolução do EAM. Existem diferenças detectáveis de morbilidade e mortalidade, após ACTP directa, no sexo feminino? Objectivos: Avaliar o perfil clínico e os resultados, em termos de morbilidade e mortalidade intrahospitalar, em mulheres submetidas a angioplastia directa por síndrome coronário agudo (SCA) com supradesnivelamento do segmento ST. Metodologia: Análise retrospectiva de 245 doentes consecutivos, 72 dos quais do sexo feminino (29,4 %), admitidos entre Janeiro de 2000 a Dezembro de 2001 por SCA com supradesnivelamento do segmento ST e submetidos a ACTP directa. Avaliámos a prevalência dos vários factores de risco cardiovascular – hipertensão arterial (HTA), tabagismo, dislipidémia, diabetes mellitus, história familiar –; antecedentes pessoais de angina, EAM e angioplastia prévios; horas de evolução do enfarte; características angiográficas e evolução clínica.Resultados: Do estudo descritivo dos dados, verificou-se que os doentes do sexo feminino eram mais idosos (67,9 ± 11,6 vs 59,6 ± 13; p < 0,001), com maior prevalência de HTA (65,3% vs 47,4 %; p < 0,05) e angina (29,0% vs 16,0 %; p < 0,05) e menor prevalência de tabagismo (27,8% vs 54,3 %; p < 0,001). A demora média (horas) entre o início da sintomatologia e a ACTP foi significativamente maior nas mulheres (6,8 ± 4,1 vs 5,4 ± 3,7; p < 0,05). Angiograficamente, o número de lesões existentes e abordadas foi semelhante nos dois grupos. Utilizaram-se inibidores da glicoproteína IIb/IIIa em 84,7% das mulheres e 90,8% dos homens. Dos eventos considerados, verificou-se maior ocorrência de complicações hemorrágicas minor (5,6% vs 5,2 %), arritmias (15,3% vs 10,4 %) e mortalidade intrahospitalar (9,7% vs 6,4 %) no sexo feminino, sem no entanto apresentar significado estatístico. A demora média intra- -hospitalar foi semelhante nos dois grupos. Conclusão: Apesar da crescente preocupação com a homogeneidade de abordagens diagnósticas e terapêuticas nos dois sexos, particularmente no acesso a terapêutica de reperfusão, a morbilidade e mortalidade são tendencialmente superiores na nossa população, sem contudo atingir significado estatístico. A idade mais avançada e a maior demora até à realização da angioplastia, podem explicar esta discrepância.
- Fractura de Stent: Caso Clínico e Revisão da LiteraturaPublication . Lousinha, A; Fiarresga, A; Cardona, L; Lopes, JP; Lopes, A; Cacela, D; Sousa, L; Patrício, L; Bernardes, L; Cruz Ferreira, RApresentamos o caso de um doente, previamente submetido a angioplastia coronária com um stent revestido com fármaco, com enfarte agudo do miocárdio (EAM) provocado por trombose tardia do stent, tendo sido detectada fractura do mesmo. Este caso vem reforçar o reconhecimento crescente da fractura como potencial mecanismo para a trombose tardia de stent, na era dos stents revestidos com fármaco. A propósito deste caso clínico, apresentamos uma revisão da literatura sobre fractura de stent, nomeadamente sobre a sua incidência, mecanismos subjacentes e complicações clínicas.
- Heterotopic Caval Valve-in-Valve Procedure for Prosthetic Migration: Two Case ReportsPublication . Grazina, A; Ferreira, A; Ramos, R; Cacela, DBackground: Heterotopic bicaval stenting or caval valve implantation (CAVI) either with non-dedicated balloon-expandable Sapien™ valves (Edwards Lifesciences) or with dedicated TricValve™ (Products + Features) has emerged as a safe and effective percutaneous treatment for high-risk patients with severe tricuspid regurgitation (TR). One technical difficulty of CAVI is the lack of native calcified structures to anchor the device, which may lead to paravalvular leak or migration. Cases summary: We describe two patients with severe TR and high surgical risk who underwent CAVI procedures, both of them complicated with device migration to the right atrium (one inferior vena cava device and one superior vena cava device). Both cases were treated with a caval valve-in-valve procedure, with good technical and clinical results. Discussion: With the recent development of several percutaneous interventions for high-risk patients with severe TR, the rate of some possible complications is not well established, and neither are the better managing strategies. Device embolization is a rare complication of transcatheter heart interventions but with potential catastrophic consequences. Less invasive strategies such as the valve-in-valve procedure may be preferable in order to avoid the exposure of these patients to complex heart surgeries with extracorporeal circulation.
- Initial Single-Center Experience of a Standardized Protocol for Invasive Assessment of Ischemia and Non-Obstructive Coronary Artery DiseasePublication . Vaz Ferreira, V; Ramos, R; Castelo, A; Mendonça, T; Almeida-Morais, L; Pereira-da-Silva, T; Oliveira, E; Viegas, J; Garcia Brás, P; Grazina, A; Cacela, D; Cruz Ferreira, RIntroduction: Coronary vasomotion disorders (CVDs), including microvascular angina (MVA) and vasospastic angina (VSA), account for significant morbidity among patients with non-obstructive coronary artery disease (NOCAD). However, protocols for CVD assessment in clinical practice are seldom standardized and may be difficult to implement. Purpose: To assess the safety and feasibility of a comprehensive coronary function test (CFT) protocol for assessment of CVD and the prevalence of different phenotypes of CVD in patients with angina and NOCAD (ANOCA). Methods: Patients with persistent angina referred for invasive coronary angiogram and found to have NOCAD were prospectively recruited and underwent a CFT. Functional parameters (fractional flow reserve, coronary flow reserve and index of myocardial resistance) and coronary vasoreactivity were assessed in all patients. Results: Of the 20 patients included, the mean age was 63±13 years and 50% were females. Most patients had persistent typical angina and evidence of ischemia in noninvasive tests (75%). The CFT was successfully performed in all subjects without serious complications. Isolated MVA was found in 25%, isolated VSA in 40%, both MVA and VSA in 10% and noncardiac chest pain in 25% of patients. Antianginal therapy was modified after the results of CFT in 70% of patients. Conclusion: A coronary function test was feasible and safe in a cohort of patients with ANOCA. CVD were prevalent in this selected group of patients, and some presented mixed CVD phenotypes. CFT may provide a definitive diagnosis in patients with persistent angina and prompt the stratification of pharmacological therapy.
- Intervenção Percutânea Urgente no Enfarte Agudo do Miocárdio por Lesão do Tronco ComumPublication . Ramos, R; Patrício, L; Abreu, A; Soares, C; Mamede, A; Fernandes, R; Gamboa, P; Timóteo, AT; Sousa, L; Cacela, D; Bernardes, L; Cruz Ferreira, RBACKGROUND: ST-elevation myocardial infarction (STEMI) with the culprit lesion in the left main artery is a rare cardiac emergency with a poor prognosis. OBJECTIVE: Review and prognosis evaluation of primary percutaneous coronary intervention (PCI) performed in the setting of STEMI with left main occlusion in a single high-volume center. METHODS: Of the 483 primary or rescue PCIs performed and followed in our hospital during a 24-month period (August 2004 to July 2006), we retrospectively evaluated those involving left main procedures and analyzed in-hospital mortality and major cardiac events (MACE) in a 12-month follow-up. We found nine patients, age 68 +/- 9 years, five male, seven with multivessel disease and two with isolated left main disease. Rescue PCI was performed in three patients and primary PCI in the others. RESULTS: Seven patients presented in cardiogenic shock and two were classified in Killip class II on admission. Inotropic drugs, intra-aortic balloon pump and abciximab were used in eight patients. Drug-eluting stents were used in six patients, bare-metal stents in two, and isolated balloon angioplasty in one. Five patients (55%) died in the hospital and the four discharged home (two of them aged 81 and 82 years) were still alive and free from MACE at 12-month follow-up. CONCLUSIONS: Clinical presentation of STEMI with the culprit lesion in the left main artery was very severe. During PCI, drug-eluting stents, intra-aortic balloon pump and abciximab were used in almost all patients. This entity had a high mortality rate even though primary PCI was performed. Those who survived had a good mid-term prognosis.
- Invasive Assessment of the Coronary Microcirculation Using the Index of Microcirculatory Resistance: Description and Validation of an Animal ModelPublication . Fiarresga, A; Selas, M; Oliveira, E; Cavaco-Gonçalves, S; Cacela, D; Carrapiço, B; Cardim, N; Cruz Ferreira, RINTRODUCTION: The index of microcirculatory resistance (IMR) enables/provides quantitative, invasive, and real-time assessment of coronary microcirculation status. AIMS: The primary aim of this study was to validate the assessment of IMR in a large animal model, and the secondary aim was to compare two doses of intracoronary papaverine, 5 and 10 mg, for induction of maximal hyperemia and its evolution over time. METHODS: Measurements of IMR were performed in eight pigs. Mean distal pressure (Pd) and mean transit time (Tmn) were measured at rest and at maximal hyperemia induced with intracoronary papaverine, 5 and 10 mg, and after 2, 5, 8 and 10 minutes. Disruption of the microcirculation was achieved by selective injection of 40-μm microspheres via a microcatheter in the left anterior descending artery. RESULTS: In each animal 14 IMR measurements were made. There were no differences between the two doses of papaverine regarding Pd response and IMR values - 11 ± 4.5 U with 5 mg and 10.6 ± 3 U with 10 mg (p=0.612). The evolution of IMR over time was also similar with the two doses, with significant differences from resting values disappearing after five minutes of intracoronary papaverine administration. IMR increased with disrupted microcirculation in all animals (41 ± 16 U, p=0.001). CONCLUSIONS: IMR provides invasive and real-time assessment of coronary microcirculation. Disruption of the microvascular bed is associated with a significant increase in IMR. A 5-mg dose of intracoronary papaverine is as effective as a 10-mg dose in inducing maximal hyperemia. After five minutes of papaverine administration there is no significant difference from resting hemodynamic status.
