- A Enfermagem como Ciência e Arte do CuidarPublication . Marinho, ACom o presente artigo pretende-se proceder a uma reflexão e questionamento sobre a eventual afinidade da Enfermagem a áreas distintas como a Ciência e a Arte. Salienta-se assim a noção de que relação interpessoal que se estabelece entre o profissional e o beneficiário de cuidados, é o que concretiza a essência da prática do cuidar (HESBEEN, 2000) e onde o apelo a requisitos essenciais a uma qualquer arte, é progressivamente mais valorizado e exigido, mas onde os conhecimentos científicos globais assumem-se como determinantes. Outra das perspectivas abordadas refere-se à escrita em Enfermagem, por ser outra vertente da profissão onde estas áreas podem encontrar pontos de convergência.
- Evidence for Nevirapine Bioactivation in Man: Searching for the First Step in the Mechanism of Nevirapine ToxicityPublication . Caixas, U; Antunes, A; Marinho, A; Godinho, A; Grilo, N; Marques, M; Oliveira, MC; Branco, T; Monteiro, E; Pereira, SDespite its efficacy, including in the prevention of vertical transmission, the antiretroviral nevirapine is associated with severe idiosyncratic hepatotoxicity and skin rash. The mechanisms underlying nevirapine toxicity are not fully understood, but drug bioactivation to reactive metabolites capable of forming stable protein adducts is thought to be involved. This hypothesis is based on the paradigm that drug reactive metabolites have the potential to bind to self-proteins, which results in drug-modified proteins being perceived as foreign by the immune system. The aim of the present work was to identify hemoglobin adducts in HIV patients as biomarkers of nevirapine haptenation upon bioactivation. The ultimate goal is to develop diagnostic methods for predicting the onset of nevirapine-induced toxic reactions. All included subjects were adults on nevirapine-containing antiretroviral therapy for at least 1month. The protocol received prior approval from the Hospital Ethics Committees and patients gave their written informed consent. Nevirapine-derived adducts with the N-terminal valine of hemoglobin were analyzed by an established liquid chromatography-electrospray ionization-tandem mass spectrometry method and characterized on the basis of retention time and mass spectrometric fragmentation pattern by comparison with adduct standards prepared synthetically. The nevirapine adducts were detected in 12/13 patient samples, and quantified in 11/12 samples (2.58±0.8 fmol/g of hemoglobin). This work represents the first evidence of nevirapine-protein adduct formation in man and confirms the ability of nevirapine to modify self-proteins, thus providing clues to the molecular mechanisms underlying nevirapine toxicity. Moreover, the possibility of assessing nevirapine-protein adduct levels has the potential to become useful for predicting the onset of nevirapine-induced adverse reactions.
- Extubações Não Planeadas. Implicações para a Prática de EnfermagemPublication . Marinho, AIntrodução: Numa lógica que apela à emergência de indicadores sensíveis aos cuidados de Enfermagem e que se traduzam em ganhos em saúde, surgem alguns esforços que visam dar resposta ao Projecto Nacional da Ordem dos Enfermeiros relativo aos Padrões de Qualidade dos Cuidados de Enfermagem. Nesta lógica, procedeu-se a uma análise retrospectiva dos processos clínicos dos 16 doentes internados numa Unidade de Cuidados Intensivos Polivalente (Lisboa), no período compreendido entre Janeiro e Agosto de 2010, que tiveram incidentes de extubações não planeadas. Entendeu-se como extubação não planeada, a exteriorização de tubo endotraqueal por anomalia ou defeito do mesmo; por manipulação indevida de profissionais ou feita pelo próprio doente. O estudo visa contribuir para a existência de momentos de reflexão e de acção, que perspectivem a criação de instrumentos que promovam sistemas de qualidade e segurança em saúde. O mesmo surgiu no âmbito da realização do Curso de Mestrado em Enfermagem, especialização em Enfermagem Médico-Cirúrgica da Universidade Católica Portuguesa.
- Formação em Gestão de Risco e Segurança do DoentePublication . Marinho, A; Barata, F; Bordalo, I; Tavares, L; Trindade, L; Lage, MJ; Ramos, SA Gestão do Risco define-se como a cultura, processos e estruturas que sustentam a gestão efetiva de potenciais oportunidades e efeitos adversos, implicando a definição e a utilização de vários instrumentos que permitem a identificação e a avaliação dos riscos (gestão de risco proactiva) e a deteção e análise de incidentes (gestão de risco reativa). A premência de disponibilizar formação sobre Gestão do Risco e Segurança do Doente a todos os profissionais do Centro Hospitalar de Lisboa Central (CHLC), criou a necessidade de encontrar múltiplas estratégias que permitam alcançar este objetivo. Desta forma a Equipa de Gestão do Risco do CHCL estabeleceu um programa de formação sobre este tema de forma a dar resposta às necessidades de formação neste âmbito em todas as áreas profissionais. Este programa de formação é realizado nos moldes tradicionais de ensino/aprendizagem com obrigatoriedade presencial, constando-se a necessidade de promover outras estratégias de formação modernas que atinjam o máximo de profissionais possível. Uma destas estratégias passa pelo planeamento e organização de formação e-learning nesta área com a finalidade de potencializar um modelo de ensino/aprendizagem não presencial.

- Impact of Treatment Modality on Vascular Function in Coarctation of the Aorta: The LOVE - COARCT StudyPublication . The effect of treatment modality on vascular function is unknown. The LOVE-COARCT (Long-term Outcomes and Vascular Evaluation After Successful Coarctation of the Aorta Treatment) study was done to compare vascular function in patients with coarctation of the aorta treated with surgery, balloon dilation (BD), or stent implantation. Methods and Results In treated coarctation of the aorta patients without residual coarctation, we prospectively compared aortic stiffness by applanation tonometry and cardiac magnetic resonance; endothelial function by endothelial pulse amplitude testing; blood pressure ( BP ) phenotype by office BP , ambulatory BP monitoring, and BP response to exercise; left ventricular mass by cardiac magnetic resonance; and blood biomarkers of endothelial function, inflammation, vascular wall function, and extracellular matrix. Participants included 75 patients treated with surgery (n=28), BD (n=23), or stent (n=24). Groups had similar age at enrollment, coarctation of the aorta severity, residual gradient, and metabolic profile, but differed by age at treatment. Prevalence of systemic hypertension, aortic stiffness, endothelial function, and left ventricular mass were similar among treatment groups. However, BD patients had more-distensible ascending aortas, lower peak systolic BP during exercise, less impairment in diurnal BP variation, and lower inflammatory biomarkers. Results were unchanged after adjustment for potential confounders, including age at treatment. Conclusions In our cohort of patients without residual coarctation, treatment modality was not associated with major vascular outcomes, even though there were some favorable vascular characteristics in the BD patients. Although this suggests that choice of treatment modality should continue to be driven by likelihood of achieving a good anatomical result, more long-term studies are required to assess the clinical significance of the more-optimal results of secondary markers of vascular function in BD patients. Although the mechanism underlying abacavir-toxicity is not fully understood, drug bioactivation to reactive metabolites may be involved. This work was aimed at identifying abacavir-protein adducts in the hemoglobin of HIV patients as biomarkers of abacavir bioactivation and protein modification. The protocol received prior approval from the Hospital Ethics Committee, patients gave their written informed consent and adherence was controlled through a questionnaire. Abacavir-derived Edman adducts with the N-terminal valine of hemoglobin were analyzed by an established liquid chromatography-electrospray ionization-tandem mass spectrometry method. Abacavir-valine adducts were detected in three out of ten patients. This work represents the first evidence of abacavir-protein adduct formation in humans. The data confirm the ability of abacavir to modify self-proteins and suggest that the molecular mechanism(s) of some abacavir-induced adverse reactions may require bioactivation.

Marinho, A; Neves, MO presente artigo tem o intuito de dar a conhecer um plano, destinado a enfermeiros que colaboram na supervisão de alunos de enfermagem em estágio, identificando algumas linhas orientadoras, de modo organizado e sistematizado, que visam a melhoria dos processos formativos. Deste modo pretende-se despertar os orientadores para algumas situações e considerações que poderão ser significativas, no sentido de facilitar o processo de ensino-aprendizagem de todos os seus intervenientes.
- Prevalence of Late-Onset Pompe Disease in Portuguese Patients with Diaphragmatic Paralysis - DIPPER StudyPublication . Guimarães, MJ; Winck, JC; Conde, B; Mineiro, A; Raposo, M; Moita, J; Marinho, A; Silva, JM; Pires, N; André, S; Loureiro, CPompe disease is a rare autosomal recessive neuromuscular disorder caused by acid α-glucosidase enzyme (GAA) deficiency and divided into two distinct variants, infantile- and late-onset. The late-onset variant is characterized by a spectrum of phenotypic variation that may range from asymptomatic, to reduced muscle strength and/or diaphragmatic paralysis. Since muscle strength loss is characteristic of several different conditions, which may also cause diaphragmatic paralysis, a protocol was created to search for the diagnosis of Pompe disease and exclude other possible causes. METHODS: We collected a sample size of 18 patients (10 females, 8 males) with a median age of 60 years and diagnosis of diaphragmatic paralysis of unknown etiology, followed in the Pulmonology outpatient consultation of 9 centers in Portugal, over a 24-month study period. We evaluated data from patient's clinical and demographic characteristics as well as complementary diagnostic tests including blood tests, imaging, neurophysiologic and respiratory function evaluation. All patients were evaluated for GAA activity with DBS (dried blood test) or serum quantification and positive results confirmed by serum quantification and sequencing. RESULTS: Three patients were diagnosed with Pompe's disease and recommended for enzyme replacement therapy. The prevalence of Pompe, a rare disease, in our diaphragmatic paralysis patient sample was 16.8%. CONCLUSION: We conclude that DBS test for GAA activity should be recommended for all patients with diaphragmatic paralysis which, despite looking at all the most common causes, remains of unknown etiology; this would improve both the timing and accuracy of diagnosis for Pompe disease in this patient population. Accurate diagnosis will lead to improved care for this rare, progressively debilitating but treatable neuromuscular disease.
