- Cirurgias de Derivação do Ventrículo PulmonarPublication . Afonso, D; Coelho, PP; Banazol, N; Nogueira, G; Rebelo, M; Freitas, I; Trigo, C; Pinto, MF; Fragata, I; Fragata, JCavopulmonary connections have been extensively used in the palliation of complex forms of congenital heart disease requiring some form of right heart bypass. We examine the mid term outcomes of pulmonary ventricle bypass operations in a single institution and performed by the same surgical team. POPULATION: Between March 1999 and April 2006, 62 patients underwent pulmonary ventricle bypass operations: bidirectional cavopulmonary anastomosis (Glenn procedure), total cavopulmonary connections (Fontan procedure) and one and a half ventricle correction in two cases. Age at operation averaged three years (range: 0.42-25 years) for the Glenn procedure and seven years (range: 3-14 years) for the Fontan procedure. There were 36 male patients (58%) and 26 female patients (42%). The most common indication for surgery was the single ventricle defect, present in 66% of patients. Associated lesions included: transposition of the great arteries in 16 patients (35.6%), bilateral superior vena cava in four patients (8.9%), situs ambigus in five patients (11%), situs inversus in another patient (2.2%), Ebstein disease in one patient (2.2) and coronary fistula in another patient (2.2%). Sub-aortic stenosis was present in one patient (2.2%). Palliative surgery was performed in all, but three patients (5%), before the Fontan procedure. RESULTS: Thirty two patients underwent bidirectional cavopulmonary anastomosis and thirty patients underwent cavopulmonary connections, total or 2nd stage. Mean cardiopulmonary bypass times were 50.6+/-21.9 minutes for the Glenn procedure and 88.5+/-26.3 minutes for the Fontan procedure. There was no intra-operative mortality, but two patients (3.2% (died in the first month after surgery; one due to failure of the Glenn circuit and sepsis and the other due to a low cardiac output syndrome and multi-organ dysfunction. Mean ventilation time was 5.2+/-1.7 hours for the Glenn operation and 6.2+/-3.2 hours for the Fontan operation. The mean length of stay in ICU was 3.4+/-2.8 days for patients undergoing the Glenn operation and 4.6+/-3.1 days for patients undergoing the Fontan operation and the mean length of hospital stay was 10.6+/-5.8 days for the Glenn operation and 19.1+/-12.6 days for the Fontan operation respectively. The mean follow up time was 4+/-2.1 years (minimum 0 years and maximum seven years), most patients being in NYHA class I. Epicardiac pacemakers were implanted in three patients due to arrhythmias. Two re-operations (6.7%) were needed, both in the same patient, after the Fontan procedure, this patient eventually died a few years after surgery. CONCLUSIONS: The immediate and mid term outcomes of pulmonary ventricle bypass operations can have excellent results. From our point of view there has been an improvement, namely in the use of the extracardiac conduit technique in the 2nd stage of the Fontan operation.
- Comparison of Outcomes in Immediate Implant-Based Breast Reconstruction: Acellular Dermal Matrix versus Inferior Dermal FlapPublication . The purpose of this study was to compare the outcomes in immediate one-stage breast reconstruction between acellular dermal matrix (ADM) and inferior dermal flap (IDF). Methods We conducted a retrospective comparative study of patients submitted to immediate breast reconstructions with an anatomical implant and ADM or IDF in a single center between 2016 and 2018. Outcomes evaluated included major complications, early complications, reinterventions, readmissions, and reconstruction failure. Simple descriptive statistics and univariate analysis were performed. Results A total of 118 breast reconstructions (85 patients) were included in the analysis. Patients in the IDF group had a higher body mass index (median = 27.0) than patients in the ADM group (median = 24). There were no statistically significant differences among both groups regarding immediate major complication, early complications, readmissions, and reinterventions. Conclusion There are no significant differences in complications between the ADM and IDF approach to immediate implant breast reconstruction. In patients with higher body mass index and large, ptotic breasts, we recommend an immediate implant reconstruction with IDF.
- Estenose Aórtica Subvalvular após Correcção Anatómica da Transposição das Grandes Artérias: Caso ClínicoPublication . Rebelo, M; Trigo, C; Paixão, A; Kaku, SOs autores descrevem o caso clínico de uma criança do sexo masculino, com 12 anos de idade, com diagnóstico pós-natal de transposição das grandes artérias com comunicação interventricular, submetida a switch arterial e encerramento da comunicação interventricular. Durante o seguimento pós-operatório detectou-se um obstáculo entre o ventrículo esquerdo e a aorta ascendente que foi, inicialmente, de grau ligeiro. No último ano começou a referir sintomatologia (cansaço para esforços moderados). Na avaliação efectuada detectou-se uma estenose aórtica subvalvular grave, tendo o doente sido submetido a tratamento cirúrgico, com bom resultado. A estenose aórtica subvalvular é uma complicação rara, que tem sido descrita em estudos de follow-up de switch arterial, sobretudo nas situações em que o defeito primário é a transposição das grandes artérias com comunicação interventricular.

We report a case of fatal meningitis caused by a highly virulent G. bethesdensis strain in an adolescent in Europe who had chronic granulomatous disease.
- Impact of Body Mass Index, Age and Tobacco Use on the Outcomes of Immediate Breast Reconstruction with Implants and Acellular Dermal MatrixPublication . Methods We conducted a retrospective study of patients who submitted to immediate breast reconstructions with an anatomical implant and acellular dermal matrix in a single center between 2016 and 2018. Outcomes evaluated included immediate complications, early complications, reinterventions, readmissions, and reconstruction failure. Patients were divided into two groups concerning each potential risk factor (BMI < or ≥25; age < or ≥ 50 years; and smokers vs nonsmokers). Simple descriptive statistics and univariate analysis were performed. Results A total of 101 breast reconstructions (73 patients) were included in the analysis. The mean BMI was 24, and the mean age was 44.5 years old. Smokers accounted for 14 breast reconstructions (13.9%). The rate of early infections, mastectomy flap necrosis, and implant removal was significantly higher in overweight patients. The total volume of breast drainage was higher in the age ≥ 50 years group. Smoking did not alter the outcomes. Conclusions A BMI ≥ 25 is a risk factor for early infections and reconstructive failure. Age ≥ 50 years is associated with a higher volume of breast drainage but does not seem to impact the success of the reconstruction. Smoking does not appear to affect the outcomes significantly in this type of reconstruction. Surgeons should consider delaying the reconstruction or using autologous tissue when patients are overweight.
- Janela Aorto-Pulmonar. Experiência de Onze CasosPublication . Freitas, I; Paramés, F; Rebelo, M; Martins, JD; Pinto, MF; Kaku, SA janela aorto-pulmonar consiste numa comunicação entre a aorta ascendente e o tronco da artéria pulmonar, na presença das duas válvulas arteriais separadas. É uma anomalia rara que ocorre em cerca de 0,1% a 0,2% de todas as cardiopatias congénitas. O objectivo deste trabalho é avaliar a forma de apresentação clínica, o tratamento cirúrgico e a evolução dos doentes com janela aorto-pulmonar assistidos num centro terciário de cardiologia pediátrica num período de 30 anos. Identificaram-se onze crianças consecutivas com o diagnóstico de janela aorto-pulmonar. Dez crianças tinham o defeito do tipo proximal e uma do tipo distal. A idade na primeira avaliação variou entre três dias e 13 anos(média=44,5±63,3 meses; mediana=três meses). A ecocardiografia permitiu fazer o diagnóstico correcto nos quatro doentes mais recentes. Sete doentes foram submetidos a encerramento da janela aorto-pulmonar por via trans-aórtica e três doentes foram submetidos a laqueação da janela aorto-pulmonar. Simultaneamente, foram corrigidas anomalias associadas significativas em três doentes: correcção de interrupção do arco aórtico em dois doentes; encerramento de comunicação interventricular e alargamento do tracto de saída do ventrículo direito com remendo de Dacron, num doente. Um doente teve seguimento médico por apresentar hipertensão pulmonar fixa. A mortalidade operatória foi de 10% (1/10) e não se verificou mortalidade tardia. O tempo médio de seguimento foi de 10±4,9 anos. Actualmente os nove doentes operados sobreviventes estão assintomáticos, sem qualquer medicação, sem defeitos residuais e sem evidência de hipertensão pulmonar. Em conclusão, a evolução das técnicas de diagnóstico e de tratamento cirúrgico da janela aorto-pulmonar nos últimos anos permite uma intervenção precoce com bons resultados clínicos.
- Multidisciplinary Outpatient Clinic of Neurocutaneous Diseases: Five-Year Experience of a Pediatric Tertiary Hospital in PortugalPublication . A multidisciplinary approach to NCS patients has been advocated, although a specific model is not yet established. The aim of this study was 1) to describe the organization of the recently created Multidisciplinary Outpatient Clinic of Neurocutaneous Diseases (MOCND) at a Portuguese pediatric tertiary hospital; 2) to share our institutional experience focusing on the most common conditions, neurofibromatosis type 1 (NF1) and tuberous sclerosis complex (TSC); 3) to analyze the advantages of a multidisciplinary center and approach in NCS. Methods: Retrospective analysis of 281 patients enrolled in the MOCND over the first five years of activity (October 2016 to December 2021), reviewing genetics, family history, clinical features, complications, and therapeutic strategies for NF1 and TSC. Results: The clinic works weekly with a core team of pediatricians and pediatric neurologists supported by other specialties as needed. Of the 281 patients enrolled, 224 (79.7%) had identifiable syndromes such as NF1 (n = 105), TSC (n = 35), hypomelanosis of Ito (n = 11), Sturge-Weber syndrome (n = 5), and others. In NF1 patients, 41.0% had a positive family history, all manifested café-au-lait macules, 38.1% neurofibromas with 45.0% being large plexiform neurofibromas. Sixteen were under treatment with selumetinib. Genetic testing was performed in 82.9% of TSC patients with pathogenic variants found in TSC2 gene in 72.4% patients (82.7% if considered contiguous gene syndrome). Family history was positive in 31.4%. All TSC patients presented hypomelanotic macules and fulfilled diagnostic criteria. Fourteen patients were being treated with mTOR inhibitors. Conclusion: Offering a systematic and multidisciplinary approach to NCS patients enables timely diagnosis, promotes a structured follow-up, and encourages discussion to outline management plans for optimal care to every patient, with significant impact on the quality of life of patients and families.
- Operação de Ross: Resultados a Médio PrazoPublication . Rebelo, M; Afonso, D; Nogueira, G; Coelho, PP; Banazol, N; Fragata, I; Pinto, MF; Fragata, JThe Ross procedure has been used in children and young adults for aortic valve replacement and the correction of complex obstruction syndromes of the left ventricular outflow tract. We report the mid-term results of the Ross procedure in a single institution and performed by the same surgical team. Population: Between March 1999 and December 2005, 18 patients were operated on using the Ross procedure. The mean age at the time of surgery was 12 years, being 12 patients male (67%). The primary indication for surgery was isolated aortic valve disease, being the predominant abnormality in 58% of cases aortic regurgitation and in 42% left ventricular outflow tract obstruction. Associated lesions included sub-aortic membrane in 3 patients (16%), small VSD in 2 patients (11%), bicuspid aortic valve in 4 patients (22%) and severe left ventricular dysfunction and mitral valve regurgitation in 1 patient (6%). Ten of the 18 patients (56%) had been submitted to previous surgical procedures or percutaneous interventions. Results: Early post-operative mortality was not seen, but two patients (11%), had late deaths, one due to endocarditis, a year after the Ross procedure, and the other due to dilated cardiomiopathy and mitral regurgitation. The shortest time of follow-up is 6 months and the longest 72 months (median 38 months). Of the 16 survivors, 14 patients are in class I of the NYHA and 2 in class II, without significant residual lesions or need for re-intervention. The 12 patients with more than a year of follow up revealed normal coronary perfusion in all patients and no segmental wall motion abnormalities. Nevertheless, two of the 12 patients developed residual dynamic obstruction of LVOT and in three patients aortic regurgitation of a mild to moderate degree was evident. Significant gradients were not verified in the RVOT. Conclusions: The Ross procedure, despite its complexity, can be undertaken with excellent immediate results. Aspects such as the dilation of the neo aortic root and homograft evolution can not be considered in a study of this nature, seeing that the mean follow up time does not exceed 5 years.

- Síndrome do Coração Esquerdo Hipoplásico com Encerramento in Utero do Foramen Ovale: Caso ClínicoPublication . Rebelo, M; Veiga, E; Machado, AJ; Pinto, MF; Kaku, SApresenta-se o caso clínico de um recém- -nascido (RN) do sexo masculino, com diagnóstico pré-natal de síndrome do coração esquerdo hipoplásico (SCEH), referenciado às 37 semanas de gestação. No ecocardiograma fetal apresentava SCEH clássico com atrésia da aorta, válvula mitral permeável e septo interauricular espessado, mas com foramen ovale (FO) aparentemente permeável. Parto eutócico de termo, PN = 2540g, necessidade de intubação endotraqueal com ventilação assistida e prostaglandina E1 intravenosa. Imediatamente após o parto verificou-se agravamento clínico com deterioração dos valores gasimétricos. O ecocardiograma confirmou o diagnóstico, levantando a suspeita de encerramento do FO, que se confirmou no exame hemodinâmico, feito de seguida. Tentativa de atrioseptostomia de Rashkind, sem sucesso. O RN faleceu, tendo a autópsia revelado SCEH com FO encerrado, seio coronário aumentado e extensas linfangiectasias pulmonares. O aumento do seio coronário levou, neste caso, ao diagnóstico pré-natal errado de FO permeável com shunt esquerdo-direito.
