- Abnormal Appearance of the Umbilicus: An Indicator of Urachal AnomaliesPublication . Espírito Santo, R; Alcafache, M; Almeida, SR; Tavares, AThe urachus is an intra-abdominal fibrous remnant of the allantois. The non-involution of the allantois can result in urachal anomalies. The abnormal appearance of the umbilicus may be a sign of such anomalies. We have observed 3 cases of term neonates with atypical appearance of the umbilical stump, all of which manifested urachal anomalies, as documented by ultrasound scan. These appearances are rarely described in the literature, and seem to regress at around 2 months. Therefore, it is important that healthcare professionals should be aware of the possible implications of atypical umbilical stumps, evaluate each case accordingly and, if an urachal anomaly is diagnosed, refer the patient to a paediatric surgery centre, as such malformations carry an underlying risk of infection or malignancy.

- Automatic Small Bowel Tumor Diagnosis by Using Multi-Scale Wavelet-Based Analysis in Wireless Capsule Endoscopy ImagesPublication . Barbosa, D; Roupar, D; Ramos, J; Tavares, A; Lima, CBACKGROUND: Wireless capsule endoscopy has been introduced as an innovative, non-invasive diagnostic technique for evaluation of the gastrointestinal tract, reaching places where conventional endoscopy is unable to. However, the output of this technique is an 8 hours video, whose analysis by the expert physician is very time consuming. Thus, a computer assisted diagnosis tool to help the physicians to evaluate CE exams faster and more accurately is an important technical challenge and an excellent economical opportunity. METHOD: The set of features proposed in this paper to code textural information is based on statistical modeling of second order textural measures extracted from co-occurrence matrices. To cope with both joint and marginal non-Gaussianity of second order textural measures, higher order moments are used. These statistical moments are taken from the two-dimensional color-scale feature space, where two different scales are considered. Second and higher order moments of textural measures are computed from the co-occurrence matrices computed from images synthesized by the inverse wavelet transform of the wavelet transform containing only the selected scales for the three color channels. The dimensionality of the data is reduced by using Principal Component Analysis. RESULTS: The proposed textural features are then used as the input of a classifier based on artificial neural networks. Classification performances of 93.1% specificity and 93.9% sensitivity are achieved on real data. These promising results open the path towards a deeper study regarding the applicability of this algorithm in computer aided diagnosis systems to assist physicians in their clinical practice.
- Degenerescência Caseosa da Calcificação do Anel Mitral: Revisão a Propósito de Seis CasosPublication . Fernandes, RM; Branco, LM; Galrinho, A; Timóteo, AT; Tavares, A; Feliciano, J; Oliveira, R; Fiarresga, A; Mamede, A; Banazol, N; Roquette, J; Cruz Ferreira, RCaseous calcification of the mitral annulus is a rare form of mitral annular calcification, whose etiology is not completely understood and which can lead to an erroneous diagnosis of intracardiac tumor. The authors describe the cases of six patients, five of them female, mean age 74.8 +/- 6.4 years (65-81). Four patients presented with heart failure, two with atrial fibrillation and five with hypertension. Round, echogenic images, 18-26 mm in their largest diameter with a central echolucent area, were identified by transthoracic echocardiography on the lateral and posterior segments of the mitral annulus. Severe mitral regurgitation was also found in four patients. Only three patients with severe mitral regurgitation and heart failure were operated on, and one patient refused surgical treatment. A caseous mass, similar to toothpaste, was obtained from the mitral annulus zone during surgery.
- Oral Infection Caused by Stenotrophomonas maltophilia: A Rare Presentation of an Emerging Opportunistic PathogenPublication . Prates, M; Fernandes, F; Proença, F; Mussá, Y; Tavares, A; Pereira, AStenotrophomonas maltophilia is an emerging multidrug-resistant opportunistic pathogen with an increasing incidence of nosocomial and community-acquired infection cases, mainly in immunocompromised individuals. Oral cavity infections are rare. To learn more about this infection, a case of oral cavity infection caused by S. maltophilia in an immunosuppressed patient under ventilatory therapy has been presented. The patient presented with multiple nonpainful erosive lesions of the tongue, palate, and oral mucosa. A smear of the oral lesions was performed that revealed the presence of S. maltophilia and Candida albicans, and the patient was treated with fluconazole and sulfamethoxazole associated with trimethoprim in accordance with the antimicrobial susceptibility testing. After 14 days of antibiotic therapy, there were almost no signs of the previous lesions.
- Pancreatite Aguda como Manifestação de Linfoma de Burkitt: em Idade PediátricaPublication . Silva, F; Paiva, M; Tavares, A; Lacerda, A; Pereira, G; Marques, A; Barata, D; Cabral, JA pancreatite é uma patologia pouco frequente em Pediatria, com etiologia mais variada que no adulto, sendo as causas tumorais particularmente raras. Os autores apresentam o caso de um adolescente de 13 anos, previamente saudável, internado na Unidade de Cuidados Intensivos com os diagnósticos de pancreatite aguda, derrame pleural bilateral e ascite. Na avaliação imagiológica apareceram imagens sugestivas de processo infiltrativo da parede gástrica. A endoscopia digestiva alta mostrou tumor infiltrativo da mucosa gástrica e bulbo duodenal com úlcera gigante, com biópsias compatíveis com Linfoma de Burkitt, confirmado por citologia e citometria do líquido ascítico. Durante a evolução houve ainda necessidade de colocação de endoprótese biliar por colestase obstrutiva de agravamento progressivo. Após estadiamento (estádio III), iniciou o protocolo FAB LMB 96 obtendo remissão completa. A pancreatite aguda e a colestase são formas de apresentação raras de linfoma neste grupo etário.
- Paragem Cardíaca em Idade PediátricaPublication . Tavares, A; Trigo, C; Estrada, JApresenta-se o caso clínico de uma criança de 10 anos com paragem cardíaca em repouso que foi reanimada, com sucesso. A investigação etiológica do acidente mostrou uma Cardiomiopatia Hipertrófica (CMH). Foi colocado um Cardiodesfibrilhador Implantável (CDI) para profilaxia secundária da Morte Súbita (MS). A Morte Súbita em idade pediátrica é uma situação rara, de muito mau prognóstico (sobrevida sem sequelas inferior a 5%) e com um impacto social significativo. A Cardiomiopatia Hipertrófica é uma causa conhecida de Morte Súbita, com possibilidade de prevenção. O caso apresentado serve para levantar questões sobre a necessidade e forma de estratégias para prevenir a MS de etiologica cardíaca em idade pediátrica.
- The Spectrum of Mutations and Molecular Pathogenesis of Hemophilia A in 181 Portuguese PatientsPublication . The spectrum of non-inversion F8 mutations in 101 families included 67 different alterations, namely: 36 missense, 8 nonsense and 4 splice site mutations, as well as 19 insertions/deletions. Thirty-four of these mutations are novel. Molecular modeling allowed prediction of the conformational changes introduced by selected amino acid substitutions and their correlation with the patients' phenotypes. The relatively frequent, population-specific, missense mutations together with de novo alterations can lead to significant differences in the spectrum of F8 mutations among different populations.
